For a reservation to be made at MediLodge of Montrose, your loved one should have …
- a Medicare Card and at least one other insurance card
- a Social Security Card
- a doctor’s or hospital’s referral notice, which includes the doctor’s orders, the History and Physical, the most recent chest X-ray, and any Mental Health Screening (MIMR) forms
- the following papers, as applicable: DPOA, Patient Advocate, Guardianship, Conservatorship
- 5 to 7 days worth of clothing
- any personal belongings
