If my family member is coming from home instead of a hospital, what must I do to get him or her admitted into MediLodge of Montrose?

- a chest X-ray, current to within 30 days
- the History and Physical – with diagnosis (medical) and Mental Health Screening (MIMR) forms
- details of your loved one’s normal daily routines
- a current list of medications and doses
- legal documents (Power of Attorney or Guardianship, for example)
